Whether you have a “gut “instinct, someone said something , or you just have a gnawing feeling…Something brought you to this page to obtain answers to some of the questions lingering in your mind.

Welcome! We look forward in assessing your hygiene department and presenting you with the feedback you need to put your mind at rest.

Here’s how the Hygiene Assessment works:

  1. Print out the Hygiene Assessment Data Collecting Form, gather the requested information, as fill out the form as completely as you can.
  2. A Performance Plus representative will contact you to schedule your Hygiene Assessment session.. The assessment sessions are conducted via phone and are from 30 to 60 minutes in duration. During this time you will find out where you practice is currently compared to the industry standards, hidden potential, and what your hygiene department could achieve if operating according to Performance Plus’ metrics.
  3. You’ll receive a Hygiene Assessment Scorecard following your session providing you with a summary of your session.
