If your answer is “yes”, simply complete this recall assessment! What do you have to lose by filling out the free assessment? What will you gain?

Here’s how the Assessment works:

  1. Print out the Recall Assessment Data Collecting Form, gather the requested information, and fill out the form as completely as you can.
  2. A Performance Plus representative will contact you to schedule your Assessment session. The assessment sessions are conducted via phone and are approximately 30 min in duration. During this time you will find out where you practice is currently compared to the industry standards, hidden potential, and what your hygiene department could achieve if recall was operating according to Performance Plus’ metrics.
  3. You’ll receive a Scorecard following your session providing you with a summary of your session.
